Direct Access
While some insurance plans require a physician referral, an increasing number of plans provide coverage for direct access to physical therapy services allowing our patients to attend physical therapy without a referral.

Medical Insurance
Sport and Spine Clinic participates in most major health insurance plans. If you have any questions regarding your insurance, give us a call prior to your initial visit.

Payment Policy
We ask that insurance co-pays be made at the time of service. It is imperative that we have a copy of your current insurance card on the day of your initial visit.

After-Hours Service
If you need to contact our office after normal business hours, please call one of our office numbers, and leave a message. We will answer your call as soon as possible.

Every appointment you attend will be carefully and skillfully designed and carried out by a licensed physical therapist. It is of paramount importance to us for patients to continue treatment with the same therapist to ensure continuity of care. Our therapists invest a great deal of time and energy to provide you with a unique and fun rehabilitation experience.

Our dedication to providing high quality care is evidenced by our efforts to set up timely initial evaluations (within 24-48 hours of first contact to our clinic). Because we are independently owned and operated, we have the ability to develop programs and services that reflect the needs of the doctors, employers, and patients that support us. We work closely with those in our local medical community optimizing patient care outcomes.
